This form MUST be completed in it’s entirety. A Parent or Legal Guardian of the Participant MUST acknowledge and consent to the Safety Protocols and Guidelines (COVID 19), Parental Player Participation Authorization , Medical and Photo/Video/Live Streaming Releases portions and Parent/Guardian Code of Ethics – by entering your name (as lawful signature) and entering the date completed.
MIAMI YOUTH BASEBALL ASSOCIATION, INC. – PARENTAL AUTHORIZATIONS AND RELEASES
PARENTAL PARTICIPATION AUTHORIZATION AND
All sports, including BASEBALL pose a risk of severe physical injury and/or illness to the participants, coaches, and spectators including, but not limited to, the exposure and possible contraction of communicable diseases (ie Covid-19), and being injured by obstacles or hazards in or around the playing surfaces and/or inherent to the game of baseball such as being injured by a batted or thrown ball, a baseball bat, or other projectile from the field, batting cages, stands, or other areas. CONCUSSION – HEAD INJURY NOTICE: A CONCUSSION is a potentially serious HEAD injury that can result in brain injury or death. Participation in baseball practices and competition (games) may result in a HEAD injury or CONCUSSION. If you suspect that the player has a CONCUSSION, he/she must be immediately removed from participation. A player should only return to play with permission and clearance from medical/health care professional, who is experienced in evaluating a CONCUSSION, the injury incurred or illness contracted. ADDITIONAL CONCUSSION information and resources are available on the MYBA website at MYBABASEBALL.TEAMPAGES.COM. I, the undersigned Parent/Guardian listed below, warrant that I am the parent or legal guardian of the Player listed above (the Player) and that I have authority to authorize these releases on behalf of the Player. For and in consideration of the Player being permitted to voluntarily participate in MIAMI YOUTH BASEBALL ASSOCIATION, INC league (including, but not limited to, any tournaments in which the Player participates (the Activity), and for other valuable consideration, the receipt of which is hereby acknowledged, on behalf of the Player, I voluntarily agree as follows: I give consent for the Player to participate in all MIAMI YOUTH BASEBALL ASSOCIATION, INC Activities, and to be rendered emergency medical treatment in the event of injury or illness and agree to be responsible for all costs associated with said treatment. In the event of a medical emergency resulting from an injury or illness, the instructors/coaches will endeavor to obtain medical treatment for the Player. The Releasees (defined below) can not assume responsibility for any act or omissions from third parties who are called to render treatment.

(collectively, the “Releasees”), with respect to any and all causes of action, claims, losses, damages, obligations, liabilities, judgments, amounts paid in settlement, injuries, losses, costs, expenses (including, but not limited to, reasonable attorneys fees and expenses), and demands whatsoever, in law or in equity, which I or the Player ever had, now has, or shall or may have against said Releasees, or which any personal representative, executor, administrator, heir, next of kin, successor, or assign of myself or the Player hereafter can, shall, or may have, against all Releasees, arising out of or relating to the Player’s participation in the Activity, including, but not limited to the Player’s death.
PHOTO / VIDEO / LIVE STREAMING RELEASE
I hereby give permission to the use of the name, likeness, images, voice, recording, biographical information, and/or other identification of the Player or myself by the Releasees for any purpose, worldwide and in perpetuity, in any medium now known or hereafter devised, without a right to prior review or approval of such use by me or the Player and without compensation or any additional consideration due or owing to me of the Player.
CONSENT AND AUTHORIZATION ACKNOWLEDGEMENT
I hereby agree and consent to all of the aforementioned, and further acknowledge that this authorization is valid for the entire 2021 seasons, including and not limited to the SPRING and FALL LEAGUES, year round practices, and all TOURNAMENTS. I do have the right to withdraw this authorization at any time by providing written notice to the Miami Youth Baseball Association, Inc. at 9372 SW 21 Street, Miami, Fl. 33165.
MYBA -Safety Protocols an Guidelines – (COVID-19) – Games and Practices
SAFETY PROTOCOLS AND GUIDELINES
Section 1 YOUTH PLAYER/PARTICIPANT
1. All Players MUST wear a facial covering while they are within the Park Complex (including the Dugouts and parking lots). The only exception is while on the plying field in pre-game warm ups and participating practices and games. Per Miami Dade County Executive Order 29-20 or the most recent Executive Order.
2. Equipment – Each Player MUST use their own personal equipment
Bags, bat, helmet, glove, batting gloves, catching gear …
Each Player’s equipment bag will be hung on the fence inside dugout and/or in front of the
bleacher (where bleacher is used as extension of Dugouts.
3. Designated Dugout (Bench Area)
Each field will have a designated dugout and/or dugout extension area.
Bleachers or bench area outside and adjacent to the Dugouts (Grapeland Park) will be
utilized in order for the Players to maintain Social Distancing.
4. Players are advised to bring their own hydration fluids (Water, Gatorade…) and snacks.
We will have limited Concessions.
5. Social Distancing of 6+ ft MUST be maintained while in the Park Complex, parking lots,
6. Players MUST bring their own personal sanitizing solution (60% alcohol) for regular hand
sanitizing. MYBA will have hand sanitizing stations throughout the facility.
7. No handshaking, fist bumps, high fives, sun flower seeds, gum…
At the conclusion of games – Teams will stand on the foul line in font of their dugout and tip
caps to the opposing team.
Section 2 PARENTS/GUARDIANS (Adults)
1. ONLY two (2) Parents/Guardians (and Siblings – supervised at all times) of a Player are
permitted to attend. It is recommended that you drop off the player and then pick them up
after the game. Parents/Guardians (and siblings) remaining in vehicles until GAME TIME or
they are notified (via text from their Head Coach/Designee) to enter the facility/seating
area(s) of the park. Siblings will be allowed but MUST remain in the seating areas, and not
allowed to play in the park and be supervised at all times.
2. All Parents/Guardians, and siblings 3 years and older (Spectators) MUST wear a facial
covering at All Times, including while exiting and returning to their vehicles in the parking
3. All Parents/Guardians MUST stay/sit and remain within the designated spectator area and
maintain social distancing (6 ft+).
Each Field at each Park shall have a designated area for spectators. Parents are NOT
ALLOWED behind home plate, behind or in the dugouts, batting cages, or playing fields
before, during or after games or practices.
Bleachers are CLOSED and off limits to all spectators.
DESIGANATED SPECTATOR AREAS – BLEACHERS ARE CLOSED
GRAPELAND PARK- Outside the playing complex along the extended foul lines and behind the outfield fences.
BROTHERS TO THE RESCUE (FLAGAMI PARK), and FERN ISLE PARK – Beyond the dugouts, from the end of each dugout (6+ft beyond the dugouts) up the left-field and right-field lines respectively. NO ONE from the end of the 1st base dugout all the way around home plate to the end of the 3rd base dugout.
TROPICAL PARK – Designated spectator area is open behind the fencing (remaining a minimum of 10ft+ from the dugouts.
4. NO TENTS, Parties, Picnics, Cookouts, Meetings, and Group Gatherings Before, during or
after the games.
5. The possession and/or consumption of alcoholic beverages anywhere in the park (including
parking lots) are prohibited by law. All Coolers are subject to inspection
6. Amplifiers, PA systems, radios, air horns, bells, whistles, music and musical instruments,
banging on bleachers, or any other artificial noise making devices are PROHIBITED.
7. All Parents/Guardians and Siblings (Spectators) Only use the designated restroom(s) – one
person at a time.
While going to and from the restrooms social distancing MUST be maintained, and you may
not stop to watch the game, approach the dugouts, or communicate with players or

Parental/Guardian Code of Ethics
Code of Ethics
Registering my Child constitutes agreement with the following Code of Ethics, and I Agree to:
•Adhere to all MYBA policies, rules, regulations, and procedures
•Avoid inappropriate behavior (fighting/arguing/cursing)
-Any person who becomes unruly or uses abusive language towards players, umpires,
coaches, league officials, or other fans will be removed from the premises (including parking
-Any adult who gets in a physical altercation on or off the field, before or after a game or
practice, will be suspended indefinitely (a minimum of one year) from the league. If the
league deems the incident involved more than one individual, severe penalties may be
invoked up to and including removal of the team from the league with-out refund or
•Be responsible for any and all bills (medical and otherwise) resulting from any injury or illness to my son/daughter, resulting from their participation in any MYBA practice, game or event.
•Acknowledge the prohibition on alcohol or illegal substances anywhere in the park (including the parking lots).
•Observe the ban on smoking anywhere in the baseball complex.
•The prohibition on the use of air horns, bells, whistles, musical instruments, radios, banging on bleachers, or any other artificial noise making devices while in the baseball complex.
•Prohibition on coolers within the baseball complex
•Always be supportive and encourage all players on the team
•Avoid commenting negatively on the performance of players, coaches, umpires,
or other parents
•Serve as a good role model for my child and others
•Treat umpires with courtesy and respect
•After each game, have a positive attitude and show appreciation for the
importance of each player’s efforts
•Save problems or concerns for the right time by talking with a coach, umpire
coordinator, staff, or board member away from the game
•Support the MYBA and its efforts however I possibly can
