This form MUST be completed in it’s entirety. A Parent or Legal Guardian of the Participant MUST acknowledge and consent to the Safety Protocols and Guidelines (COVID 19), NSID – National Sports ID Player Age Verification, Parental Player Participation Authorization , Medical and Photo/Video/Live Streaming Releases portions and Parent/Guardian Code of Ethics – by entering your name (as lawful signature) and entering the date completed.
MIAMI YOUTH BASEBALL ASSOCIATION, INC. – PARENTAL AUTHORIZATIONS AND RELEASES
PARENTAL PARTICIPATION AUTHORIZATION AND
All sports, including BASEBALL pose a risk of severe physical injury and/or illness to the participants, coaches, and spectators including, but not limited to, the exposure and possible contraction of communicable diseases (ie Covid-19), and being injured by obstacles or hazards in or around the playing surfaces and/or inherent to the game of baseball such as being injured by a batted or thrown ball, a baseball bat, or other projectile from the field, batting cages, stands, or other areas. CONCUSSION – HEAD INJURY NOTICE: A CONCUSSION is a potentially serious HEAD injury that can result in brain injury or death. Participation in baseball practices and competition (games) may result in a HEAD injury or CONCUSSION. If you suspect that the player has a CONCUSSION, he/she must be immediately removed from participation. A player should only return to play with permission and clearance from medical/health care professional, who is experienced in evaluating a CONCUSSION, the injury incurred or illness contracted. ADDITIONAL CONCUSSION information and resources are available on the MYBA website at MYBABASEBALL.TEAMPAGES.COM. I, the undersigned Parent/Guardian listed below, warrant that I am the parent or legal guardian of the Player listed above (the Player) and that I have authority to authorize these releases on behalf of the Player. For and in consideration of the Player being permitted to voluntarily participate in MIAMI YOUTH BASEBALL ASSOCIATION, INC league (including, but not limited to, any tournaments in which the Player participates (the Activity), and for other valuable consideration, the receipt of which is hereby acknowledged, on behalf of the Player, I voluntarily agree as follows: I give consent for the Player to participate in all MIAMI YOUTH BASEBALL ASSOCIATION, INC Activities, and to be rendered emergency medical treatment in the event of injury or illness and agree to be responsible for all costs associated with said treatment. In the event of a medical emergency resulting from an injury or illness, the instructors/coaches will endeavor to obtain medical treatment for the Player. The Releasees (defined below) can not assume responsibility for any act or omissions from third parties who are called to render treatment.
I hereby agree to defend, indemnify, waive, release, hold harmless, absolve, forever discharge, and covenant not to sue THE MIAMI YOUTH BASEBALL ASSOCIATION, INC., PERFECT GAME BASEBALL, USSSA BASEBALL, WORLD BASEBALL FOUNDATION, MARLINS TEAMCO LLC, MARLINS FUNDING LLC, MARLINS HOLDING LLC, MAJOR LEAGUE BASEBALL, CITY OF MIAMI, MIAMI DADE COUNTY, MIAMI DADE COUNTY PUBLIC SCHOOLS, CITY OF HIALEAH, CITY OF HIALEAH GARDENS, CITY OF DORAL, CITY OF HOMESTEAD, CITY OF MIAMI BEACH, CITY OF MIAMI LAKES, CITY OF MIAMI SPRINGS, TOWN OF CUTLER BAY, VILLAGE OF PALMETTO BAY, VILLAGE OF PINECREST, VILLAGE OF KEY BISCAYNE, MARY and DAVID ALPER JEWISH COMMUNITY CENTER, AND BOYS AND GIRLS CLUBS OF MIAMI DADE, ORGANIZERS, VOLUNTEERS AND PARTICIPATING TEAMS AND ACADEMIES and all of their respective past, present, and future related entities, parents, subsidiaries, affiliates, officers, directors, partners, owners, shareholders, members, governors, agents, representatives, contractors, subcontractors, servants, officials, employees, volunteers, participants, successors, assigns, right-holders, sponsors, insurers, and other licensees; participants; persons transporting the participants to a from activities; and any individual, group, organization or corporation under contract with THE MIAMI YOUTH BASEBALL ASSOCIATION, INC. (collectively, the “Releasees”), with respect to any and all causes of action, claims, losses, damages, obligations, liabilities, judgments, amounts paid in settlement, injuries, losses, costs, expenses (including, but not limited to, reasonable attorneys fees and expenses), and demands whatsoever, in law or in equity, which I or the Player ever had, now has, or shall or may have against said Releasees, or which any personal representative, executor, administrator, heir, next of kin, successor, or assign of myself or the Player hereafter can, shall, or may have, against all Releasees, arising out of or relating to the Player’s participation in the Activity, including, but not limited to the Player’s death.

PHOTO / VIDEO / LIVE STREAMING RELEASE
I hereby give permission to the use of the name, likeness, images, voice, recording, biographical information, and/or other identification of the Player or myself by the Releasees for any purpose, worldwide and in perpetuity, in any medium now known or hereafter devised, without a right to prior review or approval of such use by me or the Player and without compensation or any additional consideration due or owing to me of the Player.

I hereby agree and consent to all of the aforementioned, and further acknowledge that this authorization is valid for the entire 2023 seasons, including and not limited to the SPRING and FALL LEAGUES, year round practices, and all TOURNAMENTS. I do have the right to withdraw this authorization at any time by providing written notice to the Miami Youth Baseball Association, Inc. at 10940 SW 134 Ter. Miami, Fl. 33176.
MYBA -Safety Protocols and Guidelines – (COVID-19) – Games and Practices
SAFETY PROTOCOLS AND GUIDELINES
Parents/Guardians will be required to pre-register their child on-line AND
MUST read, acknowledge, and approve their child’s participation while completing all waivers and releases.
MUST agree to follow the approved Rules, Regulations and Safety Protocols and Guidelines.
MUST Agree to follow all MYBA – Safety Protocols and Guidelines – (Covid-19) as may be implemented and required by State and Local Authorities, and follow the most recently updated CDC Guidelines.
CONSENT AND AUTHORIZATION ACKNOWLEDGEMENT
I hereby agree and consent to all of the aforementioned, and further acknowledge that this authorization is valid for the entire 2023 seasons, including and not limited to the SPRING and FALL LEAGUES, year round Practices, and all TOURNAMENTS. I do have the right to withdraw this authorization at any time by providing written notice to the Miami Youth Baseball Association, Inc. at 10940 SW 134 Ter. Miami, Fl. 33176.
Parental/Guardian Code of Ethics
Code of Ethics
Registering my Child constitutes agreement with the following Code of Ethics, and I Agree to:
•Adhere to all MYBA policies, rules, regulations, and procedures
•Avoid inappropriate behavior (fighting/arguing/cursing)
-Any person who becomes unruly or uses abusive language towards players, umpires,
coaches, league officials, or other fans will be removed from the premises (including parking
-Any adult who gets in a physical altercation on or off the field, before or after a game or
practice, will be suspended indefinitely (a minimum of one year) from the league. If the
league deems the incident involved more than one individual, severe penalties may be
invoked up to and including removal of the team from the league with-out refund or
•Be responsible for any and all bills (medical and otherwise) resulting from any injury or illness to my son/daughter, resulting from their participation in any MYBA practice, game or event.
•Acknowledge the prohibition on alcohol or illegal substances anywhere in the park (including the parking lots).
•Observe the ban on smoking anywhere in the baseball complex.
•The prohibition on the use of air horns, bells, whistles, musical instruments, radios, banging on bleachers, or any other artificial noise making devices while in the baseball complex.
•Prohibition on coolers within the baseball complex
•Always be supportive and encourage all players on the team
•Avoid commenting negatively on the performance of players, coaches, umpires,
or other parents
•Serve as a good role model for my child and others
•Treat umpires with courtesy and respect
•After each game, have a positive attitude and show appreciation for the
importance of each player’s efforts
•Save problems or concerns for the right time by talking with a coach, umpire
coordinator, staff, or board member away from the game
•Support the MYBA and its efforts however I possibly can
